- QUESTION: I have already had my first balloon angioplasty, over 10 years ago,
- in but one artery and with good results. Now, with increasing angina
- symptoms, I am anticipating the need for another operation. Lately I have
- read of the use of "stents" for this procedure, and am wondering if this is
- now considered the best medical practice or merely an interesting experimental
- procedure? You can understand the basis for my interest.

- ANSWER: For the uninformed, a balloon angioplasty is a procedure used to open
- a blocked or partially blocked artery. A long tube, called a guide catheter,
- is inserted into an artery and threaded through the artery until it reaches
- the area of the blockage. A second catheter, with an inflatable balloon at
- its tip, is then passed through the guide catheter into the narrowed artery
- segment. When the balloon is inflated, the walls of the artery are pushed
- back, increasing the size of the inner passage, and allowing greater amounts
- of blood to flow more freely. This is considered to be a non-invasive
- procedure, or a non-surgical method of correcting the situation.
- Although stents were once investigated as a useful method of preventing
- arteries from becoming blocked again by a continuing build up of fatty
- deposits, results were disappointing. However, another important use for
- stents was discovered.
- Stents are scaffolding-like devices that can provide support to the walls
- of an artery, and help keep it open to the free flow of blood. In about 10
- percent of the patients undergoing balloon angioplasty, the arteries suffer a
- sudden collapse, either during or immediately following the procedure. This
- requires emergency by pass surgery to restore blood flow. The use of stents
- during angioplasty reduces both the frequency of artery collapse as well as
- its severity by half, which permits planned bypass surgery to be performed, a
- far less risky procedure than an emergency bypass operation. Certainly not
- merely an investigational procedure, the use of stents has been recommended
- for general use as an alternative to emergency bypass surgery by an advisory
- panel of the Food and Drug Administration.
